If you or a loved one needs a little assistance with activities of daily living (ADLs), we’re here to help! The experienced team at Shepherd’s Path works with residents and their families to develop a personalized care plan that addresses your unique needs and ensures you receive the right level of support as needed. Just like the rest of the accommodations at The Farms, our assisted living residence features elevated, smart design features with custom interiors, spacious kitchenettes, private bathrooms, walk-in closets, and much more.
Memory Care
Located next to our Shepard’s Path assisted living care residence, our Ruth’s Way Memory Care area was designed specifically to ensure security, provide peace of mind, and enhance quality of life for individuals with Alzheimer’s, dementia, and memory loss. Our staff is specifically trained to provide a safe, supportive environment, including an individualized care plan for each resident’s needs. Our open floor plan and secure outdoor garden area encourage social connections and interactions that provide important mental and physical activity.
Skilled Nursing Care
When residents need a higher level of medical care, our skilled nursing care team is here to provide around-the-clock attention from our highly trained, licensed, and certified staff. Our advanced therapy programs and medical care are designed to promote health even in challenging circumstances, and our Suites at Jordan River residence features accessible, private studio spaces that offer a home-like feel with comfort beds and plenty of space for friends and family to visit. We also offer respite care for short-term stays to help aid recovery or simply offer caregivers a well-deserved break.

Our 4,300-square-foot senior rehabilitation center offers a full range of highly effective orthopedic, cardiac, respiratory, and neurological rehabilitation programs. The Farms at Bailey Station is a Type A LifeCare community, offering maintenance-free independent living with guaranteed, priority access to every level of care: assisted living, memory care, skilled nursing care, senior rehabilitation, and respite care—all on one campus at a predictable, monthly rate.
For Collierville, Germantown, and Greater Memphis seniors who need additional assistance with activities of daily living (ADLs), such as bathing, dressing, or medication management, the Jordan River Health Campus offers assisted living in our Shepherd’s Path residence.
